An off-site follow up survey conducted on October 21, 2022, found that Apex Healthcare Services, LLC had corrected the following deficiency cited under 28 Pa. Code, Health Facilities, Part IV, Chapter 51, Subpart A. The deficiency was cited as a result of a re-licensure survey completed on September 1, 2022.
